Death Certificate
- Record must be for event no earlier than 1859.
- Provide request in writing in person at the office.
- Include the following:
- Explanation of the record requirement
- Full names of parties involved
- Date of record
- Cost is $25.00 for certified copy, (if no record found, refund not available)
